Form builder — the 5 tabs

A tour of every tab in the form builder — Form, Reward, Settings, Emails, and Trigger.

When you open a form in VideoTestimonials you land in the builder. Across the top are five tabs: Reward, Form, Settings, Emails, Trigger. Here's what each one does.

Form

The main canvas with every page the customer sees, stacked top-to-bottom. Click any block to open its settings in the right sidebar. Each block has a toggle in its header to include or exclude it from the form.

The page blocks appear on the canvas in this order:

  • Rating page — star rating (1–5).
  • Negative feedback page — branches here when a customer gives a low rating (1–3 stars); their written response stays private and doesn't publish.
  • Welcome page — landing screen with headline, subtitle, optional intro video, CTA button.
  • Questions page — your custom prompts. Each question can be short text, long text, yes/no, dropdown, NPS, or checkboxes (see Custom questions).
  • Private feedback page — optional always-on private text box, separate from the negative feedback branch.
  • Consent page — public / private choice for publishing.
  • About you page — name, email, photo.
  • About company page — job title, company name, website, logo.
  • Ready to send page — review screen before submission.
  • Thank you page — post-submit screen with your message, optional confetti/balloons/hearts/stars animation, primary CTA, and social share.

Reward

Pick how to incentivize submissions:

  • No Reward — default, no incentive.
  • Global Reward — same reward (a coupon code or link) given to every customer who submits. Optional toggle to limit to video-only submissions.
  • Spin the Wheel — post-submit wheel where customers spin for a chance at random rewards. Configure slices (label, probability, coupon code, winning/losing outcome). See Spin the Wheel.

Settings

Per-form behavior:

  • Form Name — internal label.
  • Maximum Video Duration — cap on recorded video length (default 120s, range 10–300s).
  • Stop collecting submissions — pause the form. Paused forms show a "form closed" message.
  • Auto-approve testimonials — skip the Pending queue; new submissions publish immediately.
  • Auto-tag testimonials — apply a fixed set of tags to every submission.

Emails

The Emails tab doesn't have per-form email settings — it links out to the workspace-level Email Requests page. Email templates and campaigns are managed globally. See Email outreach.

Trigger

The ways to share this form and get notified of submissions:

  • Share Link — the public URL to paste into email, DMs, or social posts.
  • Website Embed — an iframe snippet you paste into your HTML.
  • Notifications — Discord, Telegram, or Microsoft Teams pings when a testimonial comes in (Starter plan and up).
