Create your first collection form

Build a form to collect video and text testimonials from your customers with VideoTestimonials.

Steps to Create Your First Collection Form

1. Access the Forms Dashboard
   Navigate to the forms dashboard to manage your collection forms.

2. Initiate Form Creation
   Click the + Create New Form button in the top-right of the Forms page. A modal opens.

3. Enter Form Details

   • Name: Provide a name for your form. This helps you identify it later.
   • Description: Optionally, add a description to give more context about the form's purpose.

4. Submit the Form
   After entering the details, click Create Form in the modal. You'll be redirected to the form builder.

5. Customize Your Form
   In the form editor, you can add questions, set up video prompts, and configure settings like video duration and testimonial type (video or text).

6. Save and Activate
   Once you are satisfied with the setup, save your changes. You can then activate the form to start collecting testimonials.
